Driver Report Card

Highway safety is a top priority for the trucking industry. We want to know how truck drivers are performing on Virginia's highways. Please tell us about any experiences (good or bad) you have with truck drivers on Virginia’s roads and we will do our best to follow up with the appropriate management personnel at the driver’s company.

Please try to complete as much information as possible.  We strongly suggest that you have someone riding with you record the information or, if you are traveling alone, use a micro-cassette or other recording device to record the information. Your report will remain anonymous to the trucking company, unless you tell us otherwise in the comments section.
